Randomly loosing health and getting „debuffs“

I don’t want to call hacks immediately but check out what just happened to me on a Vanilla / Official server. Spawned in Svetloyarsk, looted some buildings and already heard some gunshots in the middle of the city too. Then, all of a sudden I have a broken leg, but it seems like I got only the icon, the debuff was not active - so I could walk properly. (You see that at the beginning of the clip).
